    The Opportunity

    Working as part of Council's Aged & Disability department, you will be responsible for assisting residents who are isolated and most vulnerable including older adults, those with disability or who are a primary carer, to fulfil their aspirations in accordance to their assessed needs and as outlined in the care plans, through their attendance in Access & Inclusion programs and activities. This role is vital in ensuring equity and access for eligible residents, and is a key role within the Aged and Disability, Access & Inclusion Unit.

    Reporting directly to Team Leader Community Participation, your responsibilities will include the following:

    • Assist with the delivery of community meals.
    • Facilitating and implementing group program activities that meet client needs, supporting them to achieve their goals and to integrate with the community;
    • Contributing to program evaluation and goal updates, including monitoring their progress;
    • Building meaningful and respectful relationships with clients and carers; and
    • Promoting acceptance, empowerment, choice and support in the community.

    Skills & Experience

    To be suitable for this position, you will possess the following:

    • Hold a Certificate III (or be working towards) in relevant disciplines, such as Community Welfare, Home and Community Services, Aged Care, Disability Care, Lifestyle Leisure, Allied Health or Community Services/Development, or equivalent work experience;
    • Knowledge and understanding of the range of issues experienced by people with disability, older persons, vulnerable individuals, and carers;
    • Demonstrated ability to support residents during innovative community programs,
    • Ability to provide support to participants as required with a focus on and commitment to person-centred practice and respectful support;
    • Experience with computer applications, including Microsoft Office and database programs;
    • Ability to drive a 12-Seater Bus; and
    • Current Victorian Driver's Licence, Level 2 First Aid Certificate and Working With Children Check
